When should preparation start?

From the moment the application is decided. A few months are enough with regular work: what is built here is reflexes, and a reflex is built by repetition, not by volume.

My child does well at school. Is this really needed?

Doing well at school and passing a competitive exam are two different things. A competitive exam is won against the clock, on questions whose shape repeats, and that shape is met nowhere in school.

Is the exam in Hebrew?

Yes, and that is one more reason to prepare: a child who meets the vocabulary of the exam on the day loses marks on questions they can solve.
